Our case management services are designed to help clients address the external, logistical, and social challenges that often accompany addiction and mental health struggles. It’s about connecting dots, removing friction, and helping each client build a sustainable, functional life outside of treatment.

What Does a Case Manager Do?

Case managers act as advocates, organizers, and resource connectors. They work side-by-side with therapists, medical providers, and support staff to ensure every client’s treatment plan is not only clinically sound, but also practically achievable.

Key responsibilities include:

  • Developing individualized support and recovery plans
  • Helping clients apply for benefits (e.g., Medicaid, food assistance)
  • Coordinating transportation or housing referrals
  • Supporting employment searches, resumé creation, and interview prep
  • Communicating with probation, courts, or child welfare (as authorized)
  • Guiding clients through legal processes or documentation
  • Assisting with appointment scheduling for medical, dental, or psychiatric care
  • Linking clients to sober living or aftercare housing
  • Ensuring follow-up & accountability for action steps outside of clinical sessions

Examples of Case Management in Action

A client living in a hotel with no ID or social security card? We’ll walk them through the replacement process, locate transitional housing options, and assist with applications.

A mother working toward reunification with her children? We’ll help her coordinate parenting classes, attend court hearings, and meet her treatment compliance requirements.

A young man with multiple court dates and no transportation? We’ll build a calendar, connect him to a sober ride service, and confirm court documents are in order.

A client finishing IOP and worried about relapse? We’ll work on building a personalized relapse prevention plan, help them transition into sober housing, and connect them to community-based supports.

Integrated with the Clinical Team

Our case managers are not separate from the rest of the treatment team. They participate in weekly staff meetings, communicate regularly with therapists and medical providers, and play an active role in treatment planning. This ensures that every piece of a client’s care, from therapy goals to job searches, is working in alignment.
