Care Coordination and Referrals

Recovery doesn’t happen in isolation. It requires a web of support–clinical, medical, social, and environmental–to help individuals navigate the complex realities of rebuilding their lives. At River City Recovery, our care coordination and referral services are designed to ensure that no client falls through the cracks and that every step of the recovery journey is connected, coherent, and supported.

We don’t just treat people: we walk with them. And that means knowing when to pull in outside resources, bridge care across systems, and advocate for our clients in moments when life becomes overwhelming.

Referral to Higher or Specialized Levels of Care

Not every person who walks through our doors is a fit for outpatient treatment, and that’s okay. In fact, recognizing this early is part of ethical care.

If, during the intake process or course of treatment, it becomes clear that a client would benefit from a higher level of care, we step in immediately to help the client access:

Detox/Medical Stabilization

We work with trusted partners to facilitate same-day or next-day admission to licensed detox programs when medically necessary.

Residential Treatment

For clients who need 24-hour structure and support, we assist with placement into inpatient or residential programs.

Psychiatric or Crisis Services

For clients experiencing acute mental health crises, we coordinate with psychiatric hospitals or emergency services to ensure safety and stabilization.

We do not view referrals as failures. They are part of good care. When a client requires more support than we can safely provide, we act decisively and compassionately to get them where they need to go.

What Is Care Coordination?

Care coordination is the process of organizing and integrating services across different providers, systems, and settings to ensure continuity of care. For clients at River City Recovery, that may include:

  • Working with hospitals, detox centers, or primary care providers
  • Communicating with probation officers, legal counsel, or family court (with consent)
  • Supporting transitions into or out of sober living environments
  • Collaborating with vocational or educational programs
  • Facilitating mental health evaluations, social services, or peer support referrals

Every client has unique needs. Some may require wraparound care; others may simply need a referral or short-term resource. Our team tailors the level of coordination based on what’s clinically appropriate and in the client’s best interest.

Downstream Referrals and Supportive Services

As clients progress in their recovery, they may need additional supports outside our clinical walls. Our case management team helps coordinate access to:

  • Sober living programs
  • Employment and vocational services
  • Continuing education or GED programs
  • Community-based mental health care
  • Peer recovery coaches or sponsors
  • Faith-based or cultural community groups
  • Family support resources
  • 12-step and non-12-step meetings (AA, NA, SMART, etc.)

We maintain an up-to-date network of vetted providers and community resources and regularly expand that network based on client needs and feedback.

Communication and Confidentiality

All coordination is done with the client’s written consent and in alignment with HIPAA regulations. We respect privacy, but we also recognize that communication with outside parties, when authorized, can dramatically improve outcomes. Whether it’s updating a sober living house about attendance, confirming compliance for legal documentation, or communicating medication updates with a primary care provider, our team handles these connections professionally and discreetly.
